Case Manager (STRTP)

The Position: The role of the Case Manager is to work in concert with the treatment team and particularly the STRTP Clinical Social Worker to liaison with the residents’ families and county workers with regards to the progress and treatment issues of the individual clients.  The Case Manager will assist in coordinating the Needs and Service Plan (NSP) and treatment/case conferences.

 

Compensation and Benefits:

The pay range we’re offering is $17.00 - $19.00 per hour depending on experience.

Qualifications:

  • Bachelor’s Degree 
  • DMH experience preferred
  • Residential experience preferred

Other Skills Required

  • Proficient in Microsoft Office Suite (MS Word, Excel, PowerPoint, Outlook) and other office-related software
  • Possess superior oral and written communication skills as well as interpersonal skills
  • Detail oriented and organized
  • Able to work independently and organize multiple projects with minimum supervision
  • Able to work as a team member
  • Able to maintain the highest level of confidentiality and professional boundaries with clients and staff
  • Able to represent the agency professionally and appropriately with all constituencies
  • Must pass pre-employment physical exam, TB and drug screening
  • Ability to drive to various sites on and off campus
  • Ability to bend, stoop, and kneel
  • Ability to physically restrain children (if needed)

 

Key Responsibilities:

  • Case Manager will maintain a caseload of clients and follow them from admission to discharge. 
  • Responsible for completing appropriate documentation, assists with the implementation of clinical interventions, and participates in an interdisciplinary treatment team for the clients on their caseload. 
  • Facilitates psycho-educational groups for the clients at the facility, when indicated. 
  • Responsible for contributing to the implementation of client behavior plans/appropriate interventions established by the Clinical Team, engaging in crisis intervention when required, and ensuring consistently high quality of client care to an at-risk youth population. 
  • The ability to implement case plans through good professional judgment while maintaining a team approach is essential. 
  • Crisis intervention and client de-escalation skills are necessary and valued.
  • Liaison with the residents’ families and county workers with regards to the progress and treatment issues of the individual clients.  
  • The Case Manager will assist in coordinating the Needs and Service Plan (NSP) and treatment/case conferences.
  • Other related duties as assigned
