The Position: We are looking for a Post Release Intake Coordinator, who is willing to embrace McKinley’s mission and H.U.M.A.N values. The Post Release Intake Coordinator is responsible for managing and coordinating case acceptance for Home Study and Post Release Services Program.

Compensation and Benefits:

The pay range we’re offering is $25.00- $29.00 hourly depending (Based) on experience.

Qualifications:

  • Bachelor's degree required in Social Sciences
  • Spanish bilingual required
  • Word processing
  • General office skills
  • Excellent reading and writing skills
  • Must pass pre-employment physical exam, TB and drug screening
  • Ability to commute to various sites
  • Ability to lift 10 pounds for filing
  • DOJ, FBI, Child Abuse Index
  • Insurability Under Corporate Automobile Insurance

Key Responsibilities:

  • Monitor phone coverage, screening of calls, and appropriate transferring of calls.
  • Maintain and enter all intake information into the Official System of Record (OSR).
  • Managing regional capacity for case assignments
  • Ensure all correspondence is documented in a timely manner and ensure all records remain secure at all times.
  • Participate in all required meetings
  • Maintain a high-level of confidentiality.
  • Ensure referrals are processed appropriately to guarantee intake criteria has been met
  • Classify, sort, input and file a variety of records, documents and materials including privileged and sensitive data.
  • Act as a liaison between the HS/PRS administrators and outside agencies as directed
  • Maintain up to date on policies
  • Utilized information gathered on Intake Form to uncover strength-based, holistic approaches for services delivery.
  • Coordinate with leadership to assign high needs and expedited referrals
  • Assess reasons for referral to assess for initial level assessment
  • Track case assignments for reporting of HS/PRS cases
  • Within 24 hours of admission, the initial assessment must be conducted. This would be offered to the UC in their preferred language.
  • Identify any immediate needs or issues, identify the severity of any medical or mental health needs, and ensure the needs are met.
  • Ensure questions are phrased in a child-friendly and culturally appropriate manner.
  • Have conversations regarding self-disclosure of unreported criminal history or any violent behaviors, which may result in the UCs transfer to another facility or affect their release.
  • Believe that people can change; demonstrate an ability to set limits and intervene appropriately to meet the needs of the children and families served.
  • Other duties as assigned.

VACCINATIONS REQUIRED: Be able to show proof of immunization or immunity to vaccine-preventable diseases transmitted by the respiratory route, including:

  • Varicella: two doses of varicella vaccine (at least four weeks apart) OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness) OR titers indicating varicella immunity OR documentation from a healthcare provider verifying previous infection.
  • MMR: two doses of MMR vaccine (at least four weeks apart) OR titers indicating immunity to all three diseases.
  • Tdap: one Tdap dose.
  • Influenza: one dose annually during flu season.
