The Position: The STRTP Facilitator provides community inclusion supports by working with a child or adolescent within the scope of facilitating CFTMs, coordinating pre meetings, obtaining necessary information from authorized representatives, clients and treatment team members.


Compensation and Benefits:

The pay range we’re offering is $85,000 - $90,000, depending on experience.

Qualifications for Employment

  • Minimum Master’s Degree with BBS registration: AMFT, ACSW, LMFT, LCSW or equivalent. Must be registered waivered or licensed.
  • Experience serving individuals with complex needs including Severely Emotionally Disturbed (SED) youth/ and families and/or youth in residential settings required.
  • Must demonstrate skills in time management, organization, and written and verbal communication
  • Experience with the mental health, child welfare and/or juvenile probation systems required.
  • Must pass pre-employment physical exam, TB and drug screening

Responsibilities:

  • Provides trauma informed team facilitation to support and enhance the efforts of youth and their families/caregivers, system workers who are part of the support team, and other individuals in a variety of environments.
  • Provides facilitation, assessment, and case management services including all related documentation for services planned and provided.
  • Collaborate with all CFTM members.
  • Complete documentation of interactions within a 24 hour period.
  • Coordinate CFTM meetings, Emergency CFTMs, Preservation CFTMs and Staffing’s for the youth In the STRTP program.
  • Complete monthly NSPs of the youth in the program.
  • Provide on call cover 1x/week working a later shift 12pm-10pm
  • Provide necessary documentation in a CFTM (I.E. CANS reports, progress reports) as needed.
  • Meet service delivery expectations.
  • Attend supervision weekly with the supervisors.
  • Attend all STRTP related training as required by the program and agency.
  • Other duties as assigned.
